Provider visit question list for GLP-1 follow-ups

A follow-up visit for GLP-1 therapy is often ten minutes. The prescriber wants to know how the dose is being tolerated, what the weight and waist have done, whether anything on the label's warning list has shown up, and whether the plan should change. You want to know the same things from the other side. This sheet puts the questions on paper so that the ten minutes are spent on answers.

What to bring with it

The dose log, the side-effect diary and the weight and waist tracker. The first question on the sheet begins "looking at my dose log and diary", which only works if they are on the desk.

The medication questions

The first is the one everyone has: stay, change, or hold? The sheet asks it in a way that invites the reasoning rather than a number, because the reasoning is what you need when the next decision comes round. The second sorts expected side effects from ones that would change the plan. The third asks for the missed-dose rule for your exact product, in your prescriber's words, so that it is written on your sheet rather than half-remembered from a website.

Two questions come straight from the labels and are the ones people most often fail to raise. The Zepbound label advises people using oral hormonal contraceptives to switch to a non-oral method or add a barrier method for four weeks after starting and after each dose increase. And both the Wegovy and Zepbound labels now warn about pulmonary aspiration during general anaesthesia or deep sedation, adding that the available data are insufficient to say whether changing fasting instructions or pausing the drug helps. If you have a procedure booked, the prescriber and the anaesthetist both need to know, and the sheet has a line for the date.

Monitoring

Then: how often visits happen, what should trigger an earlier call, and what change in weight, waist or symptoms would make them review the plan. Write the answers down; they become your thresholds.

Looking ahead
