About FormBlends Tools and Templates
What this site is, who publishes it, how the templates are built and checked, and how to reach the editorial team.
FormBlends Tools and Templates is the paperwork half of the FormBlends network: fourteen printable sheets for the parts of GLP-1 therapy that happen at the kitchen table, at the pharmacy counter, at airport security and in the prescriber's office.
What the site is
A dose log, a side-effect diary, a shot-day checklist, a question list for prescriber visits, a prior authorization checklist, an insurance appeal letter skeleton, a question list for compounding pharmacies, a travel checklist with the label storage figures printed on it, a meal and protein planner, a weight and waist tracker, a hydration and fibre tracker, a maintenance transition checklist, a supply tracker and an injection site rotation grid. Each has a page explaining what it is for and how to use it, and each can be printed or copied as plain text from the template builder.
Who runs it
The FormBlends editorial team writes and maintains this site and formblends.com. FormBlends operates a GLP-1 weight-loss program that dispenses compounded medication through licensed pharmacies. That is a commercial interest, and it is disclosed here, on the medical disclaimer, and wherever a page links to a FormBlends product or report. The pharmacy question list on this site is written to be used on FormBlends' pharmacy partners as much as on anyone else's.
How the templates are built
- Each template is a data file listing its columns, tick boxes, placeholders and sources. The on-screen sheet, the print layout and the plain-text copy are all rendered from that one file, and automated tests check that no dose column can ever be pre-filled and that no template ships without a source.
- Where a sheet quotes a figure (days a pen may stay out of the refrigerator, the deadline to file an appeal, an adequate intake for water), the figure is taken from the label on DailyMed, the agency page or the National Academies table named in the footer, on the date shown there.
- Nothing you type into the builder leaves your browser. The name and start date exist only on the sheet you print or copy.
Editorial rules
No reviews, testimonials, star ratings or named clinicians. No claim of medical review that did not happen. Doses are never suggested; the dose column on every log is for the dose as written by your prescriber. Compounded drugs are described as what the FDA says they are: not FDA approved, not checked by the agency for safety, effectiveness or quality before sale, and not interchangeable with brand products. Numbers we cannot source are not printed. Every page carries an updated date, and when a label or an agency page changes, the sheet changes and the date moves.

Corrections, broken links and requests for a template that does not exist yet go to hello@formblends.com. Quote the page URL. An error in a quoted figure is fixed as a priority and the change is noted on the page.
